Course Objectives

Learning Objectives / Learning Outcomes

The third-year internship aims to consolidate core nursing competencies, promoting autonomy, clinical reasoning, and professional leadership in complex care situations, including emergencies and chronic conditions.

At the end of the third-year internship, students will be able to:

Recognize their professional contribution within the healthcare team and engage with different professional role models.

Establish a professional relationship respecting the principles and values of both the person and the professional, particularly in situations of vital criticality and chronic illness, referring to the concepts of accountability, advocacy, caring, and cooperation.

Identify opportunities for promoting health and well-being, ensuring safety and supporting the recovery of autonomy in complex care situations.

Identify potential areas of nursing research within the domains of professional practice.

Make decisions and solve care-related problems through clinical reasoning in medium to high complexity situations.

Implement the nursing care process, performing the initial assessment, identifying nursing problems / nursing diagnoses / care needs, and defining objectives / outcomes in critical, chronic, and high-complexity care situations.

Carry out nursing interventions in accordance with guidelines and protocols, evaluating the need for possible updates through consultation of authoritative sources.

Recognize care situations that require the initiation of care pathways for patients with chronic diseases.

Communicate information within the healthcare team using appropriate healthcare and nursing documentation.

Contribute to clinical risk management in order to create and maintain a safe care environment.

Respect the different levels of responsibility and competence of healthcare and technical staff.

Ensure the right to privacy and confidentiality of health and nursing information.

Teaching Methods

Teaching methods for the internship
The third-year clinical internship is essential for advanced nursing training and takes place in accredited healthcare facilities, including highly complex and critical wards.
Clinical learning takes place through direct and guided participation in care activities, under the supervision of experienced professionals. Students are gradually introduced to complex clinical contexts, developing advanced technical, relational, and reflective skills.
Learning in the clinical context
Students participate in daily care activities under supervision:
• Observation of nursing practice in complex and critical contexts.
• Guided participation in nursing interventions.
• Gradual assumption of responsibility consistent with the level of training.
• Reflection on clinical experiences to promote professional growth.

Assessment Methods

Internship Evaluation

Evaluation is based on the observation of professional behaviors, the achievement of learning objectives, and the ability to manage complex care situations.

Evaluation considers:

Ability to apply nursing knowledge and competencies in complex contexts.

Autonomy of judgment and clinical reasoning.

Communication and relational skills.

Professional and ethical behavior.

Grading Scale

0 – behavior not acquired

1 – behavior acquired with need for help or supervision

2 – behavior acquired

Failure to achieve the expected behaviors may result in a negative evaluation of the internship period.

The final evaluation is expressed out of 20 and converted to 30 according to the Degree Program grading scale.

The evaluation is carried out by the clinical internship assistant in collaboration with the university tutor and is shared with the student.

Technical and Professional Competencies

During the third-year internship, students will progressively develop the following competencies:

1. Nursing Care Process

Collect and analyze data related to the patient, including complex and critical situations.

Identify nursing needs and problems in high-complexity care situations.

Plan, implement, and evaluate nursing interventions autonomously while monitoring outcomes.

Continuously reassess the patient’s response and modify interventions as necessary.

2. Patient Safety and Risk Prevention

Apply hygiene principles and infection prevention measures in complex care contexts.

Correctly use personal protective equipment (PPE) and follow safety protocols.

Apply ergonomic principles during patient mobilization in critical situations.

Ensure patient safety during all nursing care activities.

3. Healthcare and Nursing Documentation

Appropriately use healthcare and nursing documentation to support continuity of care and clinical reasoning.

Accurately and promptly record nursing interventions and patient responses.

Ensure that documentation supports clinical risk management and quality improvement.

Transversal Competencies
1. Cognitive Competencies

Analyze complex care situations by integrating theoretical knowledge with clinical observations.

Apply critical thinking, clinical reasoning, and problem-solving skills to make informed decisions.

Evaluate the outcomes of interventions and modify care plans when necessary.

2. Social and Relational Competencies

Establish effective professional relationships in complex or emergency situations.

Communicate clearly with patients, families, and members of the multidisciplinary team.

Collaborate while respecting roles, responsibilities, and team dynamics.

3. Ethical and Professional Competencies

Respect ethical and professional principles, especially in critical and complex situations.

Ensure confidentiality and privacy of healthcare information.

Take responsibility for professional actions and clinical decisions.

Analyze clinical experiences to identify learning needs and improve professional behavior.

Use feedback and self-assessment to improve clinical practice.

Critically reflect on experiences to support continuous learning and professional development

Evaluation of Transversal Competencies (Experimental Phase)
Starting from the academic year 2025-2026, an experimental tool has been introduced to evaluate transversal competencies mobilized during professionalizing educational activities.
This tool is currently in use at the Varese training site and is applied during internship activities by the educational tutor, with the purpose of supporting the formative assessment of the student and promoting professional development. The evaluation conducted by the clinical internship assistant in the operational units remains separate and focuses on clinical activities in real-life settings.
The tool allows for the observation and assessment of key dimensions of transversal competencies relevant to nursing education, including:
• Communication and teamwork competencies
• Self-awareness, professional role awareness, and contextual understanding
• Critical thinking, problem-solving, and decision-making
• Time and task management
• Learning from experience and professional development
• Stress, emotion, and resilience management
• Ethical, digital, and sustainable conduct
The assessment is structured across five levels of competency development: area for improvement, area for development, area of effectiveness, area of distinction, and area of excellence.
To provide students with a structured growth pathway, the evaluation by the educational tutors includes three modalities, applied in both the first and second semesters:
4. Evaluation of transversal competencies using the experimental tool.
5. Evaluation of planning in simulated contexts, to practice competencies in controlled scenarios.
6. Evaluation of planning in real contexts, to observe the application of competencies during practical activities.
The model is currently in the experimental phase, and the results of its application will be monitored and analyzed by the Degree Program in order to assess its effectiveness and the potential for stable integration into the internship evaluation system.
